This application was developed for medical personnel to facilitate access to the personal health record including the hospital visits, laboratory results, diagnosis, and drugs that be synced by the software agent which were installed in cooperated hospital. Use of the app is voluntary.

Collected data.

First, the email is important for this application to identify the users. The email was used for registration and login. Also, email was collected for password restoration.

Name, last name, position (doctor or nurse), and prefix were collected to identify the users.

Phone numbers were only collected for some functions purpose. The users be able to contact another user who have registered with phone number.

• Affiliated hospital (name and hospital ID) was collected to specify where the users work for.
• This application has a location request so that the user's location can be determined. In which the app has a location call in case the users use some function. The application is not asking for location access all the time.

Sharing data

The data collected were shared with the health care personnel for public health purposes. The data will not be shared for commercial purposes without your permission.
